Late & Administrative Fees Policy

Late Fees

Patient balances not paid within 60 days of the initial billing statement may be assessed a late fee of $20 per billing cycle.

Late fees apply only to patient-responsible balances after insurance has fully processed the claim.

Administrative Fees

The following administrative fees may apply when applicable and will be disclosed in advance:

• Returned/NSF Payments: $30

• Paper Billing Statements (Optional): $7

• Form or Document Completion: $25–$75

• Missed Appointment or Late Cancellation (Less Than 24 Hours' Notice): $75

Additional Fee Information

Late or administrative fees will not be assessed for balances pending insurance or for patients actively participating in an approved payment plan.
